Your visits may include evaluation of your current condition, medication review and management when appropriate, monitoring of relevant health information, lifestyle guidance, and an individualized plan for follow-up care.
Care is provided by Danielle Kerns, MSN, APRN, FNP-C, a Board-Certified Family Nurse Practitioner and Colorado-Licensed Advanced Practice Registered Nurse, using an evidence-based, whole-person approach.

Some conditions require laboratory testing, imaging, specialist care, or periodic in-person evaluation. Your provider will discuss these needs with you when appropriate.

When In-Person Care May Be Needed
Some chronic conditions require physical examinations, diagnostic testing, imaging, specialist evaluation, or other services that cannot be provided virtually.
If your healthcare needs extend beyond what can appropriately be managed through telehealth, your provider will discuss recommended next steps and appropriate in-person care.

Can you manage my medications through telehealth?
Medication management may be provided when clinically appropriate based on your condition, medical history, current medications, and evaluation.
Will I still need laboratory testing?
Possibly. Certain conditions and medications require laboratory monitoring. When appropriate, your provider will discuss recommended testing with you.
Do I have to be in Colorado?
You must be physically located in Colorado at the time of your telehealth appointment.
How often will I need appointments?
Follow-up frequency depends on your condition, treatment plan, medications, progress, and individual healthcare needs.
Can Chronic Care replace all of my in-person healthcare?
Not necessarily. Some conditions and preventive healthcare needs require in-person examinations, testing, imaging, or specialist care.
Do you prescribe controlled substances?
No. Renovyn Health does not prescribe controlled substances through its telehealth services.
